If this is your first case with Opal Digital Dental Lab, please complete both forms:
- Preferences Form: Register your clinic’s default CAD parameters, occlusal clearances, and scanner/milling equipment.
- Lab Order Form: Submit the specific prescription and scan files for your patient case.
Returning clinicians only need to fill out the Lab Order Form for each patient case submission.
*Your CAD preferences are securely saved on file. You only need to resubmit the Preferences Form if your clinic’s standard protocols change.
